Documentation establishing the not-for-profit corporation, "Albany-Hudson Valley Physician Assistant Program Alumni Association, Inc." also known as the PA Alumni Association, is on file with the administration of the Center for Physician Assistant Studies, Albany Medical College, MC-4, 16 New Scotland Avenue, City of Albany, County of Albany, New York State, 12208.

Articles of Incorporation - {Click here} were submitted to the NY State Department of State in 1996.

Constitution and Bylaws - {Listed here} were revised March 1994, June 1998.

US Internal Revenue Service - Form 1023 is open to public inspection, as required by section 6104, in the Internal Revenue Service's district and National Offices. IRS documentation, available at the Center for Physician Assistant Studies at Albany Medical College, was originally approved November 1996.

New York State Department of Taxation and Finance - Exempt Organization Certificate, under section 1116(a)(4) of the Tax Law of NY submitted as ST-119.2, was approved June 1996.

Financial Data - including previous Statements of Revenue and Expenses, are on file at the Center for Physician Assistant Studies at Albany Medical College. Current data has been distributed through annual meetings of the Alumni Association.
